Occupational Therapy Progress Report

A progress report is a summary report of your child’s OT program, goals, and, performance during his/her occupational therapy sessions. This is drafted by your child’s occupational therapist. This is usually needed when your child is due for a follow up check up with his/her developmental pediatrician.

*Note: Kindly request for a progress report at least 2 weeks prior to your doctor’s appointment to give your therapist ample time to prepare the report.
